Certificate of Insurance COI that covers the duration of the program and names PA Wilds Center in policyholder area
Permission form / liability waiver for parents/guardians to sign for youth’s involvement + photo permissions
Copy of commercial use agreements for DCNR/public lands, as applicable
Proof of clearances
